The Street Health office is located at 711 Pelissier Street.  Office Hours are Monday – Friday, 9:00 am – 4:30 pm. Please call for program information  519-252-3777.

The Identification Clinic assists homeless or at-risk individuals with Ontario Health Insurance Plan (OHIP) cards and when required, some supporting documents. The Street Health Identification Clinic is open Monday, Tuesday, Wednesday and Thursday from 9:00 am –  12:00 pm.

Nurse Practitioners provide medical services for individuals and families who are without a doctor and are homeless, at-risk of homelessness, new to the country or facing financial hardship.  The Street Health Medical office is open Monday, Tuesday and Thursday from 9:00 - 12:00 & 1:00 – 4:30, appointments are always preferred.

 

A Foot Nurse provides clients with thorough foot care assessments and treatments that include clipping of nails, reduction of calluses/corns, referrals for serious problems and education on self care.  Please call ahead for foot care schedule.

Supportive Services are designed to strengthen individuals and families’ while building their capacity to take more responsibility for their health and well-being. Our services provide interim support that includes education, support and advice while helping individuals and families’ access resources needed from other  programs or community agencies. It links people to resources that work to address the social determinants of health.

Supportive Services are informal, time-limited, and work along the “continuum of care” toward more in-depth case management, counseling or medical interventions.
